The cover of a digital release was changed after 6 days.
Should that now be treated as a new submission of should we add the cover to the first submission?

What is the reason for the change?

Does that matter? Well, I don't know the reason.

Hard case since it's digital, but following what we would do with physical releases in such cases I would say it's a reissue ...... I think ?

I would make a new submission, but would not call it a re-issue, they have to be at least a year apart to be considered that. Releases can have the same contents and details and just differ by the artwork, it happens all the time.
Adding some information in the release notes shall be enough to distinguish it from the first entry.
Not sure about a "new" release date though.

Ok, but adding the second cover to the first submission is really wrong then?

I'd say yes. Old artwork vs. new. Explain it in notes but keep the two separate.

Staff were asked to weigh in on this forum thread, and I'll say that it looks to me like everyone's got a good plan in this case. Digital artwork is definitely an interesting challenge more and more these days, I like the solution being proposed here -- treating the version with new artwork as a new release, and explaining it in the notes.

Okay, I kinda get the reasoning for that, at least for commercial releases.
But would you feel the same about free releases on SoundCloud which stay virtually the same (exactly same file, tracklist, SoundCloud site url) except for a changed cover? I think that would be a bit overkill.

I think this is the best solution too. Particularly as the artwork on file releases these days is actually embedded in the mp3 itself.

This is a good question. I had a file release for free recently where the commerical release of the single came with artwork. The free directly from the artist's facebook page had no artwork. If I read this correctly, I have the option now to add the artwork free file?! This aspect is not clear for me as I think my scenario leaves more room for interpretation of the decision. Some more guidance would be useful.
